What is Phase Transfer?

When you move to a new stage of school, like going from Early Years to Primary, Primary to Secondary, or Secondary to College, your plan is checked to make sure it still suits you. This helps everyone see what has gone well and what you’ve done really well so far.

In the meeting, people will talk about your new school or college, any worries you might have, and the support you still need. They’ll also think about any new things you might need help with, and any help you don’t need anymore. You can also get involved in this meeting if you want.
